Diabetes is a full-time job!

Diabetes is a challenging, serious chronic disease and its management requires constant attention, 24-hours-a-day, 7-days-a-week, 365-days-a-year.  We partner with the individual with diabetes to address their needs in a holistic way.  Some people have little or dated knowledge about diabetes self-care; sometimes, people have the knowledge, but can become overwhelmed trying to put all they have learned into practice.  Our role is to provide ongoing education, support, and motivation to our clients of all ages. We want to help you feel confident and competent to handle all of the challenges diabetes presents.

We are committed to providing long-term personalized support for people with diabetes in our community, particularly for those who often have no other place to go for information. 

Our experienced team of diabetes specialists on staff develop, conduct and oversee our program offerings, whether conducted independently or in collaboration with other agencies and provide direct, personalized care.  Diabetes professionals on our board of directors set the programming direction and provide additional sources of expertise to ensure that each client receives the highest quality of care.

Diabetes Partnership has  diabetes educators on staff at our office, at Camp Ho Mita Koda, or at numerous locations throughout the community, our educators are ready to help you with your diabetes needs!

Stop in the office for free risk assessment, attend a presentation, join a group, talk with our social worker or nutrition educator, or sign up for classes.

Our patient education and support services for those with type 1 or type 2 diabetes are provided through direct services and community outreach, including:

Junior and Family Nutrition Wellness Bootcamps

direct assistance with diabetes supplies or insulin

“Ask an Educator” by phone or online

diabetes risk assessments.

 

 

Camp Ho Mita Koda includes our programs for children with diabetes and their families, including residential and daylong programs at our 72-acre facility in Newbury Township.
