Diabetes Self-Management Education (DSME)

Diabetes Partnership of Cleveland works to educate people with diabetes about self-management to give them the skills to prevent the overwhelming complications of the disease so that they are able to live healthy, productive lives. 

Diabetes education and support is a collaborative process through which people with or at risk for diabetes gain the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions.

Sessions are held at the Diabetes Partnership of Cleveland and various locations in the community. Participants must have a physician referral, type 1 or type 2 diabetes and must be 18 years of age or older. Registration is required.

Check with your insurance provider to see if Diabetes Self-management Education or Medical Nutrition Therapy is covered.  If you have trouble paying for the sessions let us know and we can discuss options.

The classes include group and individual sessions with a registered dietition and/or nurse who are Certified Diabetes Educators.

    • control of high and low blood sugars
    • home blood glucose monitoring
    • diabetes medications -- how do they work?
    • stress management
    • exercise
    • carbohydrate counting
    • menu planning, dining out, reading food labels, portion sizes
    • foot and skin care
    • sick-day management
    • long-term complications of diabetes

American Association of Diabetes Educators Accreditation

Diabetes Partnership of Cleveland is accredited by the American Association of Diabetes Educators (AADE). This allows residents in and around Cleveland increased access to critical diabetes education and support services.
