Community Outreach

We help clients focus on taking charge of their disease through participation in group discussions, developing relationships that provide peer support, learning appropriate problem-solving techniques, setting realistic goals through personal action plans and acquiring coping skills.  

Because we incorporate successful research-based practices in all our programs, our process almost always increases motivation of our clients to manage their disease.

In addition, we conduct periodic community assessments and know transportation is a significant barrier to treatment and education.  Therefore, we are committed to working with clients in their own neighborhoods as much as possible.  Our education staff is out in the community to provide needed diabetes self-management education and support programs to all populations.
